1. This application must be properly completed as applicable in ink or by typewriter.
2. Application must be accompanied by the following:
A.) SURVEY indicating exact location and dimensions of lot, existing and proposed buildings, set-backs from property lines and relationship to adjoining property.
B.) Two complete sets of plans and specifications describing the nature of the work to be Performed, the materials and equipment to be used and installed, and details of structural, mechanical, electrical and plumbing installations.

EXCEPTIONS NOT REQUIRING AN ARCHITECHT'S OR PROFESSIONAL ENGINEERS STAMP OR SEAL.

• Farm buildings and other buildings used solely and directly for agricultural purposes.
• Single family residential buildings 1,500 square feet or less.
• Alterations costing $20,000 or less, if these alterations do not involve changes affecting the structural safety or public safety of the building or structure.

If the plans, drawings and specifications are unclear, incomplete or vague, the Code Enforcement Official may require a Stamped Set of Plans.

3. The set of plans, specifications and survey submitted herewith shall become a permanent part of the building permit file and retained by the municipality.
4. Any changes to the approved plan must be approved by the Code Enforcement Official.
5. New York State requires the following before a building permit may be issued:
A.) Proof of Workers Compensation (Form C 105.2 or U-26.3)
B.) Proof of NYS Disability Insurance (Form DC 120.1)
C.) Affidavit of exemption to show specific proof of Workers Compensation Insurance coverage for a 1,2,3 or 4 family owner-occupied residence (Form BP-1)
